There is a fair amount of good equipment on the mud that is not a matter of killing common mobs to get. Much of the better equipment for fighting classes is quest based. You need to find mobs who will offer the quest, and do what they ask. Some quests are simple. Others are more complex. Sometimes the reward is worth the work. Other times it is not.
I have broken the quests into major quests, town quests, other quests and just plain nice gear that may not be easy to get.
The two major quests in the game are currently the Stratz series of quests and the Shiro series of quests. Both quest require pursuing quests within quests to complete. You are required to fight big mobs to be given an assignment, which you complete, then return, fighting same mobs again to continue. Both of the major quests are intricate and require extensive knowledge of the mud to complete.
Current minor quests include rumania and abbey
Mystic Adventures 3 introduced the concept of a 'town quest' where each temple town had a quest for a piece of nice equipment. I have completed the quests in Olympus and Nippon. I am not aware of any other temple town quests.
Object 'opal bracelet' is type armor, extra flags none. Weight is 1, value is 200, level is 49. Armor class is 1. Worn:Takeable Wrist Affects damage roll by 7. Affects mana by 90.
Opal bracelet quest is straightforward although frustrating. Kill the cam'ed, sanced, breath 'tree frog'. Get key from him. Use key to unlock the old still in the room with the purple enfan. Get purple key. Purple key unlocks the case in jewelry with the opal bracelet. Case 'blows up' doing several thousand hp damage through sanct about 3 times out of 4.
So, just open the chest and die. Bracelet is rarely in the chest so it takes a few dozen deaths to pop a bracelet. I have not bothered to pop one in a year or two.
Object 'earrings queen gold' is type armor, extra flags glow hum invis. Weight is 2, value is 16960, level is 54. Armor class is 0. Worn:Takeable Ears Affects armor class by -15. Affects damage roll by 4. Affects hp by 54.
Queens earring is a nice item in it self. It can also be given to King Roland, and he will drop a kings cape. Queens earrings repop in a pile of bones in the basement of Kerlak Castle. You need the key from King Kerlak to open the bones, and the key from Pewy to get to the bones. Door can be passed if you want to avoid killing Pewy. The most difficult part of the quest is waiting for the item to pop, and killing the 3 mil xp, sanc'ed skeleton guarding the room.
Update 1998: the quest for kings cape indicates you need are to provide proof that Kerlak is dead. This has been changed. Current quest is simply to go to basement and kill skeleton until earring pops (about once every 2-3 days).

There is a wandering mob called 'adventurer' who has the clue to this quest. Find him and he will tell you everything you need to know. Basically, you get queens earring. Give queens earring to King Roland (this is proof that king kerlak is dead) and he will drop a kings cape.
Eyes of death are not really a quest. You get a few keys from mobs in Travellers Inn to get to Gyson's room where you fight him. The equipment is good enough that it warrants an entry.

Sceptre of Deception is on water crawler at bottom of well in Travellers Inn. You get key from delic, get chest key from wrestler, go upstairs, get into room, unlock chest, get yard key from chest, which gets you out the side door where you kill archer for water key which opens chest at bottom of well where you get key to go down to kill water crawler to get the sceptre. Nice mana user hold item
